Many of my autoimmune patients ask the same question the week they’re diagnosed: why now? Why 34, why 41, why after two decades of feeling fine? The honest answer is that autoimmune disease was never a single event. It’s the endpoint of years of accumulated load the immune system absorbed quietly before it produced a symptom you could name.

The Root Primers: What’s Actually Loading the System

Autoimmune disease doesn’t start with one trigger. It starts with a set of primers that, stacked together, push the immune system past the point where it can self-correct. These are the top 7 that I see in clinical practice:

  • Recurrent infections — repeated viral or bacterial exposure trains the immune system toward molecular mimicry, where antibodies built against a pathogen start recognizing the body’s own tissue.
  • Low micronutrient status — zinc, selenium, vitamin D, and vitamin A are functionally low. They are raw material for T-cell regulation and immune signaling.
  • Chronic stress — sustained cortisol elevation suppresses natural killer (NK) cell activity.
  • Sleep depletion — immune repair and antibody regulation happen during sleep. Cut the sleep and you cut the repair window.
  • Immunosenescence — the immune system ages, and an aging immune system loses precision — slower to respond appropriately, faster to misfire.
  • Low dietary protein — insufficient amino acid intake limits raw material for antibody production and tissue repair.
  • Poor gut health — this one connects to nearly everything else on this list. More on that below.

Autoimmunity Doesn’t Happen Overnight — It Accumulates for Years

The diagnosis is the last event in a process that typically runs a decade or more, not the first. It moves in a predictable sequence:

  1. Genetic predisposition. The blueprint is present, but a blueprint isn’t a diagnosis.
  2. Environmental load accumulates. Infections, toxic burden, chronic stress, and gut dysbiosis compound year over year.
  3. The first autoantibodies appear. Silently, often years before any symptom.
  4. Subclinical inflammation sets in. This is the phase where patients start to feel “off” without a name for it.
  5. Clinical diagnosis. The point where inflammation and tissue damage finally cross the threshold that shows up on labs or in symptoms.

This is why “why now?” is the wrong question. The more useful one is: how much load did this system carry before it finally tipped? By the time a diagnosis lands in your 30s or 40s, the process has usually been running for awhile.

Why Women Carry 80% of the Autoimmune Burden

Sex hormones are among the most powerful modulators of immune function we have, and that cuts directly against women. Estrogen amplifies innate and humoral immune responses, including antibody production — useful for fighting infection, less useful when the immune system is already primed to misidentify self-tissue. Postpartum is one of the highest-risk windows for autoimmune onset for the same reason: it’s a period of rapid immune recalibration.

Testosterone runs the opposite direction — it suppresses pro-inflammatory cytokine production, which appears to be at least part of why men carry a meaningfully lower autoimmune burden.

Treating the Immune System Means Treating All Systems, Not One

Vitamin C, glutathione, and a short course of steroids treat a symptom or a short phase. They don’t touch the systems that set immune tone in the first place. Immune function is downstream of at least six interconnected axes, starting in the gut: roughly 70% of immune cells reside in gut-associated tissue, so gut integrity isn’t adjacent to immune health — it’s primary. Sleep runs a parallel axis. Immune cytotoxicity peaks at night, and sleep is what keeps that nocturnal immune spike regulated instead of running unchecked.

Stress physiology is next. Cortisol suppresses NK cell and T-cell function, and chronic cortisol elevation means immune dysregulation, higher infection susceptibility, and more autoimmune flares. The nervous system feeds in directly: the vagus nerve modulates gut immune tone, and emerging research points to BDNF playing a role in regulating microglial activity in the brain.

Thyroid hormone enhances innate immune surveillance — which is part of why Hashimoto’s is the most common autoimmune condition globally. And metabolic health closes the loop: insulin resistance drives pro-inflammatory cytokine production, obesity itself functions as a chronic inflammatory state, and blood sugar control is immune medicine.

Where Detox Fits In — And Why It’s Not Always About Kale

A well-supported detox is a whole-body event, not just a food swap. Done correctly, it moves the needle across every axis. It lowers total toxic burden — when toxins are cleared safely and completely, not just mobilized and recirculated — which in turn lowers systemic inflammation as a direct downstream effect. It resets HRV, a measurable marker of autonomic and immune recovery, and resets cortisol rhythm, restoring a normal diurnal curve instead of a flat or inverted one.

On the input side, it resets food intake by removing the sugar and processed foods that drive inflammatory signaling, and resets the gut by expanding microbial diversity through detox-supportive foods rather than restriction alone. It resets sleep, re-anchoring circadian rhythm in a way that also improves glymphatic clearance in the brain. And it repairs immune nutrient pathways, restoring the raw materials — zinc, selenium, vitamin D, vitamin A, protein — that support immune regulation.

The Takeaway

Autoimmune disease is cumulative. Every protocol that only addresses food is treating one slice of six interconnected axes. A well-supported detox lowers the cumulative burden across all of them at once — which is the only lever that actually changes the trajectory, not just the symptom.
